Absentee Ballot Request Form

Please note that Absentee Ballots must be returned to the Clerk's Office by 8:00 p.m. on Election Day. Postmarks are not acceptable.

Birth Certificate Request Form
Fee: $10 for first copy; $5 for each additional copy

Death Certificate Request Form
Fee: $10 for first copy; $5 for each additional copy
